Your Family’s Journey to Better Health Starts Here
We’re honored you’ve chosen Elevated Life Family Chiropractic! We know visiting a new office can come with a lot of questions, so we’ve created this page to walk you through each step of the new patient process. From your first appointment to beginning regular care, you’ll know exactly what to expect before you arrive.
Our goal is to make your experience as smooth, comfortable, and educational as possible so you can feel confident as you begin your family’s journey toward better health.
Before Your Initial Visit
Once you’ve scheduled your first appointments with Elevated Life Family Chiropractic, our team will send you the appropriate new patient paperwork to complete before your visit. Taking the time to thoroughly fill it out helps your doctor better understand your health history and allows us to make the most of your time together.
If you have any questions while completing your paperwork or before your appointment, please don’t hesitate to contact our team. We’re happy to help!
Your Initial Visit
Your initial visit is designed to help us understand you, your health goals, and how your nervous system is functioning. It consists of a consultation, a neurological examination, neurological scans, and x-rays when clinically indicated. The consultation, examination, and scans are performed in our Ammon office, while x-rays are taken in our Shelley office. Although we typically complete the consultation and examination before taking x-rays, scheduling availability may occasionally require that order to be reversed. Don’t worry—our team will coordinate everything for you and make the process as convenient as possible.
Your consultation begins with a one-on-one meeting with your doctor to review your or your child’s health history, discuss your concerns, and talk about your goals for care. We will then perform a comprehensive neurological examination, which may include range of motion, reflex testing, muscle and sensory evaluation, followed by our CLA INSiGHT neurological scans to assess how your nervous system is functioning. Examinations for children vary based on the patient’s age and developmental stage. If you have any questions or concerns, please don’t hesitate to ask before your visit. Click here to learn more about our Insight-Scanning technology.
If spinal x-rays are clinically indicated (with the exception of pregnant mothers), they will be taken at our Shelley office. Since x-rays are performed on Tuesday or Thursday mornings, they may be scheduled either before or after your examination, depending on availability. Our team will explain exactly what to expect every step of the way.
Your consultation, examination, and neurological scans typically take 60–75 minutes. If x-rays are needed, that appointment generally takes about 15 minutes.
While we love seeing kids in the office, we recommend leaving them at home for this appointment whenever possible so we can give you our full attention. If the appointment is for your child, we also ask that siblings stay home, if possible.
Appointment Holding Fee
To reserve your appointment, we require a holding fee that will be applied toward the cost of your first visit.
Because appointment availability is often limited and new patient appointments are typically booked about two weeks in advance, we ask that any cancellations or rescheduling requests be made at least 48 hours in advance. Appointments canceled with less than 48 hours’ notice may result in the holding fee being retained.
Thank you for helping us provide timely care to all of our patients.
Your Report of Findings & First Adjustment
Once we have reviewed your health history, examination findings, neurological scans, and any necessary x-rays, your doctor will meet with you to explain exactly what we found and discuss your personalized recommendations for care. This visit is an opportunity to better understand how your nervous system is functioning, what may be contributing to your concerns, and how chiropractic care can help you move toward your health goals.
If you decide our office is the right fit for you and your family, you’ll receive your first adjustment during this visit.
Please allow approximately 60 minutes for your Report of Findings and first adjustment.
We encourage spouses or supportive family members to attend this appointment. A lot of important information is shared, and having another set of ears can be incredibly helpful. It also gives your support system a better understanding of your care recommendations and how they can encourage you throughout your health journey.
Beginning Your Care (Chiropractic Assistant Report – CAR)
At this appointment, you’ll receive your adjustment and then meet with one of our chiropractic assistants to review our office policies, answer any remaining questions, and make sure you know exactly what to expect throughout your care. We’ll go over scheduling, your care recommendations, payment options, and other important details so you feel confident moving forward.
Once your initial visits are complete, you’ll be able to schedule your regular adjustments at either our Shelley or Ammon location based on your care recommendations and appointment availability. From here on out, your visits are quick, efficient, and designed to fit seamlessly into your routine.
We’re excited to partner with you on your health journey and look forward to helping you and your family reach your highest potential.
Watch Your Special Appointment Video
Our Special Appointment video can be watched before any of the appointments outlined above, but we recommend watching it as soon as possible. This presentation introduces you to our office, explains our approach to chiropractic care, and helps you better understand the important role the nervous system plays in your overall health.
We believe that an informed patient is an empowered patient. Watching this video before beginning care allows us to spend more time answering your specific questions and discussing your unique health goals during your appointments.
Watching this video and completing the accompanying survey is one of the steps toward qualifying your family for our Family Assistance Pricing and adjustment packages, which can provide significant savings for your family. After watching the video, please complete the short survey to confirm your completion and help us ensure we’ve answered the most important questions before you begin care.
You should receive a text message with a link to the survey. If you don’t receive it, simply use the link below or let our team know, and we’ll be happy to send it again.
